Customer Forms

Thanks for your interest in our daycare and boarding services.  

We just need a few forms to be completed in order to commence your service with us. 

Some of the forms are a little detailed but the more information we have, the better we can care for your pup.  We are also bound by our license to capture certain data and maintain records should our licensing council, Wycombe DC, wish to inspect them. 

Information and Consent Form (required)

This form provides us with everything we need to know for the care of your dog.  It also includes some mandatory consents we need from you in accordance with the new Animal Welfare Regulations 2018.

Veterinary Release Form (required)

This form grants us permission in case of an emergency situation where we need to take your pet to the vets for treatment.  It enables you to specify your wishes in case we need to act promptly. 

Medication Consent Form

This form only needs to be completed if your pet is on medication which we will need to administer to them.

The form tells us about the medication and dose your pet requires.  Keeping a separate form allows us to act quickly if we need to inform a vet in an emergency situation.

Thanks for your interest in our dog walking service.  

If the only service you are likely to require from us is dog walks please Please complete the information form for dog walks only to ensure a happy and safe walk.  Tell us about your dogs behaviour, recall and let us know what consents we have for your dog being off lead, in a car….etc. If you may require other services in the future – daycare or boarding – you may want to use the eWoof information & consent form to save time in the future.

We also thought it would be useful to give you the option to complete a veterinary release form. We use these for our daycare and boarding guests and in the event of an accident could help to save time to get your pup emergency treatment quickly.  It’s optional but we think it’s always good to plan ahead.

Please complete the information and consent form to ensure we meet your expectations while looking after your cat.  Tell us about food, litter tray changes, your cat’s coming and goings and anything else we should know.
